​Policies

• COVID: You must wear a mask in the clinic. We also ask that only essential persons attend the appointment. If you have an in-office visit and are experiencing flu-like symptoms, please call the office to reschedule your appointment. 

• Missed appointments: If you need to change or cancel your appointment, you must do it at least 24 hours in advance in order to avoid  a missed appointment fee. If you need to change or cancel your appointment, see our contact page.
   

New patients

Please arrive 30 minutes early and bring your photo ID, insurance card, list of current medicines, any MRI or CT scans, and your treatment and medical history. Save some time by clicking the button below and completing the New Patient forms online, or printing out the form pdf below and filling it out before coming in for your appointment.
