Regardless of whether you're visiting our Mesa AZ neurology clinic, our Chandler AZ neurology clinic, or visiting with us remotely via telehealth, the information below will help you prepare for your appointment. 

​Policies

• COVID: You must wear a mask in the clinic. We also ask that only essential persons attend the appointment. If you have an in-office visit and are experiencing flu-like symptoms, please call the office and request a telehealth visit. 

• Missed appointments: If you need to change or cancel your appointment you must do it at least 24 hours in advance in order to avoid  a missed appointment fee. If you need to change or cancel your appointment, see our contact page.

Telehealth

If you have a telehealth appointment, you should get an email from us before your appointment with a link to click on for the appointment. 

New patients

Please arrive 30 minutes early and bring your photo ID, insurance card, list of current medicines, any MRI or CT scans, and your treatment and medical history.
